GLP-1 visit prep checklist

Use this to organize what happened between appointments and the questions you want to bring to a licensed clinician. It is a memory aid, not medical advice.

Medication routine

  • Medication name exactly as prescribed
  • Current prescribed dose and schedule
  • Dates and times you recorded doses
  • Any missed or delayed routine entries to discuss
  • Current medication list, including non-GLP-1 medicines and supplements

Symptoms and timing

  • Symptoms you noticed and when they occurred
  • How intense or disruptive each symptom felt
  • Whether a pattern appeared around meals, hydration, movement, or dose day
  • Anything new, worsening, or concerning that needs direct clinical review

Daily patterns

  • Meal and protein consistency you actually recorded
  • Hydration entries and days that were harder
  • Movement, workouts, or changes in usual activity
  • Weight records, if you chose to track them
  • Sleep, energy, or other context you want your clinician to know

Questions for the visit

  • What should we review based on the symptoms I recorded?
  • Are there labs, vitals, or other measurements you want before the next visit?
  • Which daily patterns are most useful for me to keep tracking?
  • What should prompt me to contact the care team sooner?
